Extrait

In chronic heart failure (CHF) changes in heart function are associated with alterations in peripheral perfusion and glucose metabolism. Whole body insulin resistance is prevalent in CHF patients with either ischemic heart failure or idiopathic dilated cardiomyopathy . Recent in vitro and animal studies strongly suggest that telmisartan, unlike other angiotensin II receptor blockers, acts as a partial peroxisome-activated receptor-gamma (PPARy) agonist. PPARy agonists are used to improve insulin sensitivity and lipid profile in type 2 diabetes. Thus we compare the effect of the investigated substances Telmisartan and Candesartan on insulinsensitivity by a frequently sampled oral glucose tolerance test and a frequently sampled intravenous glucose tolerance test. We hypothezise, that the effect of Telmisartan is more pronounced compared to Candesartan.

Critère d'inclusion

  • both investigated drugs are well established in the treatment of heart failure or hypertension respectively. As the investigateed Patients have this conditions according to the inclusion criteria, treatment will be given as clinical appropriate. Background thearpy will be given according the guidelines of the ESC over the study period
